- Description:
- This form, Form DD 4-D, is used for the registration of rental dwellings in the United States. Landlords must register each rental dwelling unit separately, whether occupied or vacant. The form should be completed in triplicate and submitted to the Area Rent Office. Tenants must also sign the form and indicate if the equipment and services included in the rent are accurate.
